Short-term therapy after a senior leaves the hospital
Being able to leave a hospital does not mean a patient is ready to go back to regular life. It takes about two to four weeks for a senior to recover and regain his abilities. Community hospitals help by caring for the senior during this time.

Get a referral from a doctor at the public or private acute hospital where the patient is staying. The community hospital will then update you on the transfer.
Prior to the admission, the community hospital will conduct means-testing to determine if the patient is eligible for subsidies. If you qualify, subsidies are available from Medifund, MediShield Life, and ElderShield. You can find community hospitals using the E-care Locator.
