How you can request a copy of your medical report
The request for medical report is only applicable for students who were seen at the Student Health Centre (SHC) and only for the duration they were seen at the SHC.
To apply for your medical report, you will have to submit written consent for the release of your medical information. If you are below 21 years of age, you will need your parent’s written consent.
Note: The release of any medical information is subject to approval by the Health Promotion Board.
There are several ways to obtain your Medical Report:
a. Email us at Contact_YPS@hpb.gov.sg attaching scanned copies of the student's NRIC (front and back). If the student is below 21 years of age, please also attach scanned copies of the student's Birth Certificate and the NRIC (front and back) of the parent who is consenting to the request. Please ensure that you provide your contact number with your request as you will be contacted once the report is ready for collection.
b. Visit us in person at Student Health Centre (Level 3), HPB.
A fee of $50 is charged for every Medical Report request, payable upon self-collection at the Student Health Centre. The fee charged includes the doctor's fee to review the patient's medical records and the administrative cost to prepare the report.
Generally, it will take 2 to 3 weeks to process the medical report, from the time the request is submitted.
You will be contacted once the medical report is ready for collection. Please ensure that you provide your contact number with your request.
You can collect it in person at the Registration Counter, Student Health Centre, Level 3. For more information on SHC’s operating hours and the payment modes, you may visit: Student Health Centre.
Please present your NRIC when you collect the medical report.
If you are picking up a Medical Report on a patient’s behalf, please present an authorisation letter from the patient, with your name and NRIC detailed in the letter.
