*The Child Health Plan initiative will be progressively rolled out starting from Primary 1 to Primary 3.

What’s in the Health Plan?

Each year, as part of the annual health screening in schools, our experienced doctors and nurses will create a customised Health Plan for your child, with your child. The Health Plan includes:

Personalised for Every Child

Your Child's Health Plan is tailored to meet your child's unique needs.
This personalised plan is an important part of the annual health screening conducted in schools, ensuring that every child receives expert guidance on their journey to wellness.

How Does It Work?

To create your child's personalised lifestyle prescription, the school health doctors and nurses will refer to your Child Lifestyle Questionnaire* inputs to:

Engage
Discuss with your child to understand their unique needs and interests.
Assess
Identify specific lifestyle areas where improvements can be made.
Prescribe
Provide a list of goals specifically discussed with and agreed upon by your child to work on for the year.

*The Child Lifestyle Questionnaire is an online form that will be provided to parents prior to the school health visits to collect information about their child’s lifestyle habits.

The lifestyle prescription focuses on four critical areas, ensuring a holistic approach to your child's health:

Using the Child Health Plan

1. Refer to your Child’s Health Plan

See what was discussed with the school’s screening team as your child may have already identified an area where he/she is keen to work on. Or you can identify an area which you and your child are keen to work on together.

For example, your child has committed to exercising three times a week by riding a scooter for 30 minutes each session. With that, you can join him/her by jogging slowly alongside while they ride the scooter.

If your child does not have a habit of eating fruits daily, both of you might like to work on eating a serving of fruit every day.

2. Plan and stick to it

Make a pinky promise on when and where both you and your child will take the action.

For example, every Monday, Wednesday and Friday, you can have an apple with your child after reaching home from school or work, and then go for a walk together after dinner.

3. Bump up the praises

Encourage and reward your child’s efforts with positive reinforcement.

Use plenty of verbal praise and reward progress with star stickers, making each active moment feel valued and celebrated.

Getting started is always the hardest. Take it slow, trust the process, it will get easier.
